So the old "dying with condition X" interpreted as "dying because of condition X" problem

This is something we've always had to warn our researchers about. In some databases we use, we have an indication of patient death, but none of them include cause of death.

For a few acute items you can make the immediate connection, e.g. MI, but otherwise you're just assuming the correlation is causation.
