With ETOPS and current engine technology, there's no way to make a four engine plane operate as efficiently as a two engine one.
hmmmmm....a twin engine 747 while mind warping to look at it, I guess that might work but the 747 design is so old that to realistically build something, it would have to be inspired by the 747 as opposed to dusting off the physical blue prints.
A current version would have different wing shape, partially for advancements in materials and wing shape, partially for a two engine design.
All new materials and thus a different from the ribs up fuselage design to account for the different weights and strengths of modern materials.
All new glass cockpit with a redesign probably for only a two person cockpit.
New fly by wire or at least partially so
So now that's a white paper design and there's nothing to have been saved in design by starting with the 747.
Though I guess consumer mind-share could be invaluable so there's that?
