I'm not a parent, but I think the medical reasons justify it. If the person is biologically able to, it's not a nonsensical question.

I would see it as jarring if it was asked to a 7-year-old (biological) boy.
That said, it needs to be asked non-judgementally. Which, well, in the US may not be the case.
