probably the most frequent minesite incident is one of those running over a light vehicle.
I was at a mine a while back that had them all driverless, which was a weird experience. every vehicle with a driver had an extra screen that had all the autonomous trucks marked on it with their projected path for the next 10 or so seconds and a perimeter marked around them. cross either of them and the autonomous vehicle stops and needs a manual reset.
they were also way more efficient, easier to maintain, easier for the digger operators to get in position (they could tap a screen rather than say "move three feet left and two feet back.") and they never do meth. a lot of minesite truck drivers do meth.
