To expand - what insurance covers does not equal what insurance pays.

What they DO NOT cover - will usually be written off by any provider they have a contract with.
What they cover will then be broken down into other contractual adjustments, deductibles and co-payments.
Hence - you owe more than the different between the total and what's covered.
