Simple answer is that it is an html attribute that can be defined to display a text string inside an image frame if the image fails to load. -- (edited)

But it's also a machine-readable attribute, so can also be used for accessibility -- to provide information about an image to someone using a text-to-speech reader to access a webpage, for example.
