My ex-wife used to be on the board of a charitable organization that provided mental health services (including housing and help with homeless people.

They estimated that 50% of the homeless population (at least in Chicago) were dealing with mental health or substance abuse issues (and most occurred before the homelessness, though the homelessness can certainly exacerbate the problem). A meaningful portion of the mental health issues were pretty severe (like schizophrenia) and the people were both resistant to taking meds and preferred being on the streets to being in shelters. It's very complex.
