Teams exist to win. Players want to make a ton of money.
You win by going max effort because 97mph is harder to hit than 91mph. So, if you suck at 91, you're getting sent down.
Teams have no incentive to let a guy throw at 90% efficiency and give them 200 innings of lesser-quality pitching and the pitchers have no incentive to throw to a 4.75 ERA which gets them DFA'ed.
The risk/reward scale changed from even 20 years ago, and it's not going back.
The only way this might have some level of help is to drop the mound more to level ground since part of the early Tommy John process is throwing off flat ground. Sounds like that's let strain than normal pitching...plus, more offense.
