Heath insurance is the only kind of insurance I know of where there are entire departments devoted to coming up for reasons to deny your claim.

And when cornered about the fact that the procedure you need hasn't been considered investigational for 20 years, the rep on the phone will likely reply: Oh no no, we're not telling you that you can't have it done. We're just saying that we won't pay for it.
