there is no positive to outweigh the negatives of smoking.

people, especially in the u.s. where the negatives have been unhidden/broadcast for decades now, who choose to start are choosing to deal with consequences. the only teeny counter is that it's still legal, but that only goes so far.
when you choose to use something that is known to have been created to be addictive and, with that, almost always negatively impacting eventually, there's not really much victimizing going on, imo. that's what leads to more consequences than for other kinds of situations.
everything else you mention can be used in non-unhealthy ways, and issues only present for segments of the population who use them. sometimes people have no control over what happens when they use whatever for "acceptable" reasons - people who choose to smoke can't say that, as there really isn't an acceptable, rational reason to start using something that almost always causes negatives.
