I agree with your first point, why would he allow himself to be in his office that night if he knows he's going to be attacked there.
I'm guessing the woman isn't in America and is getting the ultrasound in another country. The only reason to have an ultrasound at night would be if she was in distress of some kind and that doesn't appear to be the situation.
