Fringe question (spoils)

When the shape-shifter got Charlie in the first episode... when did it happen? Do we have to believe that he changed forms in the few seconds between gunshots and when the others arrived to see "Charlie" standing over the woman nurse's body? Also where did that body come from? I dont' know, the whole setup confused me
