and in addition to the factors you cite, the root cause of much of it is $$$

and the resultant political establishment power system that rewards Republicans for sticking to their party line, and rewards Democrats for bucking theirs (see Joe Lieberman)
There's also the factor of the nature of their respective ideologies, in a simplistic sense, that the GOP is trying to pull the country back to a known past, while the Democrats (or the liberals in the Democratic Party, at least) are pulling in different directions towards a unknown future.
