What the spammers do is (essentially) copy the captcha image, then present it to a steady supply of users for them to type it in, in exchange for something - usually free porn.
So once the user types it in, they check it against the captcha that the spam target is presenting, and if it works, then they're gold. If it fails, they reload the captcha and give it another try - it's usually pretty fast once they get the will-crack-captchas-for-porn site enough traffic.
"Crowdsourcing", sort of, rather than outsourcing.
