To answer the inside question, the answer is simple: HIPAA. The biggest hallway gossipers about patients are doctors.

And many of them have been caught more than once by patient's families or friends. You can make something up in this scenerio in a heartbeat (pun!)

For the rest of it, make up some story about treating an old man that you assumed was poor and homeless who turned out to be a wealthy old man working in his garden or something. That's pretty common too, the moral being--treat all your patients as if they are your mom or dad, or even a grandparent. The difference in a doctor's behavior and treatment are quantatively different than those who are judgemental about their patients.
