Like Amanda says, it's the patient's responsibility to get their records in the right place.

And like Bery says, if you've been dissatisfied with him for 3-4 years, just find a new doctor!

An apology couldn't hurt, but I wouldn't say it's mandatory. I'm sure the doc has heard worse. :)
