they messed-up two things. not bringing the salad before the entree (that's the server's issue, probably); and not including an item listed for the

item (cook's mistake, with follow-up weirdness from server -- that's like, "oh, you wanted cheese on your cheeseburger?")
while I think 5 checks for a party of 5 is excessive (hello, cell phone calculators for splits), there are 5 checks at 5 separate tables - what percentage of mess-ups overall is ok?
