It has never been a problem, but I use plastic bottles so there's no concern about shrapnel.

My recipe is pretty simple. The only way I can really over-carbonate it to add too much sugar and leave too much head space. The head space controls the amount of oxygen available for the carbonation process. Once the oxygen is gone, the carbonation stops.
