the short answer is that if a guardianship petition is filed, the court would appoint a doctor to evaluate her

it doesn't *sound* like she is at the point where a court is likely to find her incapacitated and put someone in charge of her (that's a pretty high standard to reach in Maryland)

the main reason i suggest getting an initial consultation (which should be free) with an attorney is so that you and Will at least have an idea of what all the potential issues in this case are. then, at that point, you can make a decision about what the best next step might be for everyone concerned
