Well, here is the explanation that I got from the doc but now I need to put the pieces together. The portal hypertension causes build up of fluid in

the abdomen (ascites). Since the fluid can't really go anywhere, it starts pushing on the organs. If the muscles holding back the organs are weak you get a hernia.
The patient had TWO hernias, inguinal and umbilical.
However... I can't really find any data really confirming this.. so either I am not searching under the right terms or I am just wrong.
But I swear the doc said it was a complication from his ascites.
