3 happened. Rhodes warned him. 1 is because everyone knew Stark would be at Monaco. He didn't randomly pick a place. He selected the target where Stark was shown to be going to on Russian television.

2 is a valid criticism, but an attempt to explain it is made. He had it out for the expo and he never went back to his office afterward, because first he was dying and then he didn't have the job any more. Pepper had asked him to come get his stuff and that's why it was there.

If we want to nitpick the film, the most accurate one is that Rhodes can immediately fly the suit after it took Stark all of that training in the first movie.
