Two non-negotiable things you MUST do: 1) Contact a lawyer AND your accountant. 2) Contact the agency in CA responsible for collecting taxes.

Regardless of whether the guy is legit, you'll NEED legal advice.
If you have ANY outstanding debt owed to the State, the State WILL have record of it and can confirm whether the guy who contacted you is actually their representative.
Do NOT fill out and return the form you are being sent until you have done those two things.
