I'm not convinced anything that happened on the island was real. Jack woke up in the stream, stumbled to the bamboo forest (where we saw a bloody shoe hanging from the tree), and lay down and died. However, the series was all about the blink of Jack's eye opening - the nano-second to him but 6 years to us - and it closing as he died as a result of the plane crash.

It was all (a) in Jack's mind or (b) on a metaphysical plane of existence.
