I don't think it's just in Jack's head. I think it's purgatory in a metaphysical sense. They were all 'on the island in purgatory,' but it wasn't an island in the Pacific. It's not "here." It was there, on the island in purgatory, that they all met each other and redeemed themselves.
That way, it's plausible that there was a Jacob/MIB that watched over Purgatory and turned it over to Hurley/Ben.
Something like that.
