Because the shoe was there in episode 1, season one.

This is Jack *exactly* where he was in that episode.

IMO, it's no accident that the show opens on his eye opening and closes on his eye closing as he lays in exactly the same spot with exactly the same wound.

This says to me that Jack died in the initial crash. Everything that comes after takes place in limbo/purgatory. The only events that actually happened to him are the events shown in flashbacks.

I think the same goes for every character. If it happened post crash, it was the collective consciousness of the souls from the plane and the souls on the Island creating a reality to help them move forward and let go of their lives to move on to whatever is next.
