Anyone have experience using the UPS/Paypal combo?

A user in Canada bought an item from us. Paypal charged $17 in shipping for a label and we sent the item to them. When it arrived, they were informed that shipping actually cost $40 and that they couldn't have the item until they paid that amount. I have no idea how/why this would happen.
