I can't remember if anyone had gone beyond the Hotel subconscious level yet when the van tumbled. If no-one had, then the van tumble should have acted as a kick to wake them from the Hotel level because no-one had gone further than that yet.

That's a good point about the elevator. Perhaps the charges moved the elevator. Charges were placed to break the cables and on the frame or near it. So it may not have fallen so much as been propelled by the explosion.
