I think what will trip people up is (spoil)

that they expect Leo's character to be a reliable narrator.

He is fundamentally an unreliable narrator. Yes, he believes what he's saying, but the premise of the film makes his beliefs unreliable as a gauge of truth.

I really do like this film.

It's pacing was a bit off and that's why I can't give it an A+. If the bit with the tumbling van happens in the right order with other events then it is a solid A. If the van tumbling down the embankment happens in the wrong order then the film is slightly (but importantly) flawed.
