My personal theory is also that the "real world" in the movie is just a dream and the wife got out when she killed herself.
If the wife were actually dead and the real world wasn't a dream then the top could be a true tell for Cobb. The totem only tells you whether you're within your own dream or somebody else's. If the wife is dead then there would still only be one person who knew.
Another question might be whether they even had the totems before Cobb and his wife went deep. It's possible that they didn't have those yet and the top in their dream was just the wife's projection of the truth, and that that was what gave Cobb the idea for the totems in the first place. That's just pure speculation on my part, I don't recall anything that supports that idea one way or another.
