I think it does matter.
Unless his wife is dead in the top level realty, then that top is hers and would be influenced by her desires and beliefs, superseding Cobb's beliefs.
It is an intriguing question and the film is dense enough that I can't answer it with any certainty after just one viewing.
My feeling is that the ending is cut off where it is to make you give thought to what you saw and try to suss out what will happen to the top and that if you do so there is a definitive answer.
On first viewing, my gut says the top spins and does not fall. That it's a dream and it is *the* reality that Cobb has decided should be his reality whether it is true or not.
