Not only is it his wife's totem, not his, it was in other people's hands. They make the point early on that you shouldn't hand your totem over to anyone else. The guards have the totem, as does Watanabe. He can't even be sure it is the same one.
Also, how do they get out of the deepest level to wake up on the plane? There's no kick or tipping, they just stare into each other's eyes?
I also thought the van going down the embankment would have affected their equilibrium.
I'm surprised at the negative reaction of some to the ending. It seemed that with all of the uncertainty about the different levels, it was leading up to the ambiguous ending all of the time.
I also thought the top level was collapsing at the end and that was why the totem looked like it might fall over, leading to even more levels of confusion. It falls, but he's in another level, with an unreliable totem.
