Here's an Inception question about the who was dreaming which dream and who was left behind (spoils)

So it's pretty much accepted that the dreamer is "the one left behind." The chemist is dreaming the first dream and is the one driving the van because everyone else is asleep and in the second dream (where he is conspicuously absent). The 3rd rock kid is the dreamer in the 2nd dream because he isn't at the ice fortress. Here's where it gets weird...

In the 3rd dream, supposedly the mark is the dreamer, but he not only dies in the dream (which should end it since the others were sharing his dream, unless you can dream after you die) but he goes one level deeper. In fact everybody from the ice fortress dream goes one level deeper, except the Brit, which is why I initially thought he was the dreamer.

So who was dreaming the 3rd dream, and why did it break the rules that had previously been established.
