I will, first, admit that I may be wrong. I was wrong about a few things the first time around, and this seems to be the type of movie in which multiple explanations of events are possible.
However, I believe Fisher is the subject in all three dreams. His subconscious fills the larger vault with the projection of his father, and the smaller vault with the projection of the paper fan (which was in the picture that his father broke while sick, when Fisher thought he didn't even recognize it).
But - I distinctly remember them mention that level 3 was to be Fisher's dream, and that's what has me in knots.
