i just always pick something they like but get them the more luxurious version of it, that's they'd never spring for.

sweater? get cashmere.
into a hobby? get the thing that seems like a little too much money for what it is, even though she wants it.
pajamas? buy silk ones.
that sort of thing.
it feels decadent, no matter what you pick (it doesn't need to be a big item, just depends what she's into.)
