I don't think that's the case. I think it's the fact that they pay NO attention to anything you say.

once they push their little cheeseburger button, they don't know who the fuck you are.
So when you get to the window, they probably didn't actually make a note of what kind of toy you wanted and they looked in your car and made that decision.
McDonald's workers don't give a shit. They don't care if they gave you the right food...the Happy Meal toy ranks somewhere below the sandwiches on the "things the Mickey D's drive thru window kid cares about" list.
