Ok I figured it out. I had one image which I must have inserted directly from Visio, rather than saving down to jpg and then inserting.

When I hovered over it, it gave me the option to 'edit this visio diagram'. I deleted it and the size of the doc immediately dropped to under 500k. Re-inserting a jpg which was exported from Visio has kept the size at a reasonable level.
